About Spiritual Care Advisor – Hospice (PRN)

ENTRY REQUIREMENTS

Graduate of accredited seminary or school of theology (B.D., M.Div. or equivalent theological degree). Experience working with death and dying, individuals/family/caregivers.

Hospice/home health experience preferred with experience working as member of interdisciplinary teams.

Demonstrated knowledge of, and commitment to, the Hospice philosophy of care.

Proven ability to communicate effectively verbally and in writing. Computer literate.

Comfort in an interfaith setting with the ability to accept different lifestyles, cultures, beliefs and values. Ability to network with community clergy and congregations.

Meets Home Health and Medicare standards as evidenced by criminal background check and fingerprinting.

Possesses current Michigan motor vehicle license, ability to drive a car, an insured vehicle capable of transport to patient homes or other offices in various weather conditions.

Meets MHH established requirements for proof of health status.

SPECIFIC DUTIES

  1. Supports the Mission, Vision and Values of Munson Home Health.

  2. Embraces and supports the Performance Improvement philosophy of Munson Home Health.

  3. Promotes personal and patient safety.

  4. Uses effective customer service/interpersonal skills at all times.

  5. Meets established productivity standards.

  6. Assures spiritual assessment of patients/families/caregivers in the Hospice program and that appropriate services are rendered in a timely manner.

  7. Facilitates the development in the interdisciplinary team plan of care to meet identified spiritual needs; works in close collaboration with local clergy when desired by patient.

  8. Provides direct spiritual care to patients/families/caregivers when requested in accordance with the ongoing needs and wishes of the patient/family. Assess response to and satisfaction with care.

  9. Serves as liaison and support to community chaplains and spiritual counselors.

  10. Documents direct services and ongoing communication with community chaplains and spiritual counselors in the electronic clinical record.

  11. Attends patient care conference and interdisciplinary team meetings as a core member of the interdisciplinary team. Assists the team developing and updating the Plan of Care.

  12. Provides the interdisciplinary team with consultation, education and support on spiritual care.

  13. Recruits community chaplains, spiritual counselors, and spiritual care volunteers to meet patient/family/caregiver needs by developing community contacts and offering education to congregations about Hospice care.

  14. Provides for funeral or memorial services, religious services of prayer, worship, and rituals for patients/families as appropriate and is available to families during the bereavement period.

  15. Plans periodic memorial services along with the Bereavement Coordinator to meet the needs of personnel, volunteers and community clergy/spiritual counselors working with the Hospice team.

  16. Assists in supervision of spiritual care volunteers with assigned to patients/families/ caregivers.

  17. Offers patients/families of different philosophies and religious beliefs opportunities to discuss and share thoughts, beliefs, and values.

  18. Meets with individual staff regarding personal spiritual/pastoral issues affecting their ability to function effectively and is available to staff for bereavement needs.

  19. Employees with email are required to maintain proficiency in the basic functions of the program and are also required to regularly check email and keep calendars up to date.

  20. Performs all other duties as assigned.
