About Social Worker

SUMMARY Assists patients and families in restoring social, emotional, and financial functions related to illness and disability using a wide variety of techniques and resources. Works in collaboration with Case Managers with primary responsibilities related to family dynamics, relationships, adjustment problems and complicated community referrals. May interface with patients and families at any point on the care continuum.

ELIGIBILITY REQUIREMENTS

Current license as Social Worker in the State of Michigan required with either a baccalaureate or a master’s degree in Social Work. This degree must be from a social work program that is accredited by the Council on Social Work Education (CSWE)

Two to three years of experience in clinical practice at a health care setting highly desirable.

Exposure and/or experience in pre and/or post-acute environments of care with current knowledge of community resources.

Advanced communication and interpersonal skills including conflict resolution, constructive feedback and problem solving abilities

Working knowledge/experience in utilization management, managed care, and payer issues that may impact the course of care.

Strong interview, assessment, organizational and problem solving skills. Ability to identify appropriate community resources on assigned caseload and to work collaboratively with patients, families, multidisciplinary team and community agencies to achieve desired patient outcomes.

Possesses excellent interpersonal communication and negotiation skills in interactions with patients, families, physicians and health care team colleagues.

Knowledge of services available in the local and regional communities, especially for patients with limited or no funding source. Possesses excellent communication and negotiation skills. Must demonstrate patience and tact when working with patients, families and members of the healthcare team.

Strong analytical and PC skills.

Ability to work independently as well as to develop collaborative relationships with physicians, families, patients, interdisciplinary team and other community agencies.

Demonstrates the knowledge and ability to connect patients and families with necessary services, both inside and outside the MMC facility.

SPECIFIC DUTIES 1. Supports the Mission, Vision and Values of Munson Healthcare

  3. Has basic understanding of Relationship-Based Care (RBC) principles, meets expectations outlined in Commitment To My Co-workers, and supports RBC unit action plans.

  4. Uses effective customer service/interpersonal skills at all times.

  5. Provides support, guidance and/or crisis intervention to patients and families in need of special assistance, adjustment to illness or catastrophic event, adjustment to discharge plan, support at death, or any other social or emotional needs as determined by the Social Worker or the healthcare team

  6. Provides evaluation and intervention in cases involving child abuse/neglect, domestic violence, abuse/neglect of the elderly or disabled, institutional abuse/neglect and sexual assault. Reports findings to appropriate agencies.

  7. Acts as a resource to the hospital staff.

  8. Serves as a resource person and provides support and intervention related to treatment decisions and end-of-life issues.

  9. Participates in discharge planning activities for complex patients, in order to ensure a timely discharge and to provide appropriate linkage with post-discharge care providers. Refers appropriate cases to the Complex Discharge planner

  10. Assists patients/families in obtaining financial and physical resources to meet their needs, including assistance with application for state and federal financial resources.

  11. Staffs and manages family reception area for all disasters/code triage.

  12. Leads and facilitates debriefings as indicated.

  13. Assesses needs and provides follow-up of patients/families in need of social work intervention.

  14. Assesses patient’s environment and factors that help or hinder patient’s ability to participate in and continue with treatment plans.

  15. Assists with assessment of need for discharge planning services in collaboration with Case Manager.

  16. Participates in community education, support groups and community service activities.

  17. Assists with maintenance of a community resource file.

  18. Tracks and reports Progression of Care delays and other statistical data as requested by the department manager.

  19. Participates in quality improvement plan activities and any other departmental research or studies as requested by the department manager.

  20. Demonstrates commitment to the Department’s goals and to the organization’s mission, vision, and values.

  21. Adheres to the social work profession discipline code of ethics.

  22. Fosters positive internal and external customer relations.

  23. Works in accordance with applicable state and federal law and the uniqueness of reimbursement systems.

  24. Maintains knowledge about and acts in accordance with law and procedures regarding patient confidentiality and release of information, legal requirements for reporting abuse, and advance directives.

  25. Demonstrates priority setting and time management capabilities.

  26. Fosters effective work relationships through conflict resolution, constructive feedback skills and problem solving abilities.

  27. May assist in training and orientation of new department employees and students.

  28. Performs other duties and responsibilities as assigned.
