The Registered Nurse Case Manager – Home Care serves as the primary care nurse for a caseload of home health care patients; providing case management and clinical care. Responsible for coordinating with all disciplines involved in providing quality, cost-effective care through direct care under the direction of the physician. Performs the initial assessment and mutually establishes the plan of care with the patient, family, and the physician. Establishes outcomes to be achieved within the episode of care. Accountable for ensuring compliance with federal, state, and local laws and regulations, as well as accrediting organization and professional nursing standards.


  • Admits patients in accordance with admission criteria.

  • Completes comprehensive OASIS assessments.

  • Develops an individualized plan of care and initiates services in collaboration with the patient, and family; under the direction of the patient’s physician.

  • Coordinates patient care and services from admission to discharge through effective clinical conferences, communication, scheduling and documentation.

  • Ensures that the patient receives care that is medically necessary for meeting specified outcomes within the episode of care.

  • Evaluates and revises the plan of care as indicated with necessary physician orders.

  • Supervises the patient care by paraprofessionals.

  • Manages and documents patient care, services, and resource utilization that result in quality based effective patient outcomes and cost.

  • Assumes on-call responsibilities, including weekend and holiday rotation.

  • Functions as a preceptor to new nurses.


  • Associates Degree of Nursing from an accredited educational institution.

  • Licensed Registered Nurse in the State of Michigan.

  • Must have the ability to work near fumes such as wound odors, home odors and cigarette/cigar/pipe smoke.

  • Valid Michigan driver’s license.

  • Dependable transportation and documentation of current auto insurance (must maintain valid auto insurance).


  • Home health care experience strongly preferred.
