Immediate full time Program Coordinator for our Persons with Disabilities group home in Traverse City, Michigan. This position has a $12.75/hour pay rate.

Job Summary

Coordinates full range of activities for persons served to ensure implementation of individual program plans including assigning staff duties as part of daily operations in the home.

Duties and Responsibilities

  • Assist in planning, conducting and scheduling staff education classes, on the job training and orientation programs.

  • Assist persons served with personal care, implementing individual plans of service as needed.

  • Collaborate with assigned case manager/social worker and other professional support staff on the development and implementation of individual program plans.

  • Review and evaluate daily documentation of incident reports and program assignments.

  • Participate in establishing and meeting home’s objectives relating to persons served.

  • Assign staff duties daily; monitor and ensure quality of service meets all division, agency and regulatory standards.

  • Assume supervisory responsibilities in the absence of the home manager.

Job Qualifications

Education, Training, and Licensure/Certification

  • Satisfactory completion of DCH Residential Program Training Requirements.

  • Attain and maintain all required certifications (i.e., CPR, First Aid, Blood Borne Pathogens, etc.).

  • Complete required Community Mental Health (CMH) Group home training.


  • Two years’ experience working with persons with developmental disabilities.

Knowledge Skills and Abilities

  • Ability to communicate effectively orally and in writing.

  • Effective leadership skills.

  • Ability to carry out written, oral and diagrammatic form.

  • Ability to document accurately and completely.

  • Ability to read, write and speak English language with proper use of grammar, punctuation and diction.

  • Ability to perform basic mathematical calculations.

  • Ability and willingness to work with vulnerable adults.

Additional Work Requirements

  • Valid unrestricted driver’s license with good driving record.

  • Proof of automobile insurance if required by program/center.

  • Weekends and evening hours required. Ability to work flexible schedules.

  • On-call responsibility for assigned area required.

Physical and Mental Requirements

  • Ability to safely lift and transfer persons served.

  • Ability to push persons served in wheel chair weighing up to 250 pounds on a frequent basis, often for long distances.

  • Near visual acuity.

  • Hand-eye coordination.

  • Hand and finger dexterity.

  • Ability to move freely about.
