ENTRY REQUIREMENTS Graduate of an accredited school of nursing. Nursing Baccalaureate Degree preferred.
Currently licensed in the State of Michigan.
Minimum of (3) years experience in acute care setting within the past (5) years.
Recent experience should include care of surgical patients, patient assessment, patient teaching, admission planning and discharge planning.
Utilization Management and Case Management experience preferred. Candidate should have recent experience/knowledge concerning current medications and testing procedures.
Assertiveness, organizational skills, effective time management, flexibility and self-motivation are essential. Excellent communication skills and ability to work with the public. Basic computer skills required, i.e., use of Medstar system, word processing knowledge. Completion of QES training/involvement in previous quality work groups. SPECIFIC DUTIES PROFESSIONAL NURSING PRACTICE Â· Completes patient assessment profile according to guidelines. Â· Identifies environmental, psychosocial, biophysical, self-care, educational and discharge planning factors affecting patient/family needs. Â· Obtains and utilizes information from patient/family and/or referring facilities to facilitate continuity of care and initiate appropriate referrals. Â· Identifies and prioritizes patient’s actual/potential needs in collaboration with patient/family. Â· Initiates a plan of nursing care as pertinent to the advanced registration area: in collaboration with physicians, other clinical disciplines, patient/family; including the initiation of the discharge plan for continuing care needs. Â· Documents all components of the nursing process in a concise, legible format consistent with nursing policies/procedures. Â· Communicates with patient/family regarding patient care at the level of the patient/family understanding. Â· Demonstrates respect for patient’s rights by: a) maintaining strict confidentiality of patient’s information, b) responding to his/her right to information, and c) acting as a patient advocate. QUALITY MANAGEMENT Â· Maintains knowledge of current rules and regulations related to the Utilization Management program including Pre-Admission Review and Discharge Planning. Updates knowledge as necessary and appropriate. Â· Performs admission review process, objectively applying the appropriate screening criteria. Â· Makes clinical decisions independently in situations which require adaptation of routine protocols. Â· Identifies actual/potential risk to patients and others and initiates action to minimize or eliminate risk. Â· Participates in quality assessment and improvement activities including collecting data, formulation of conclusions and development/implementation of action plans. Â· Seeks out and proposes opportunities for continuous quality improvement in clinical practice, patient outcomes and service initiatives, utilizing nursing research, education and CQI findings. Â· Reports incidents which may indicate area of actual/potential problems or opportunities to improve. Â· Uses CQI processes to assist in problem resolution. EDUCATION Â· Assesses patient/family/significant other learning needs, initiates teaching protocol considering: a) patient/family readiness to learn, b) barriers to learning, c) resources available to the patient discharge, d) anticipated length of stay, e) preventative health maintenance and f) self-care abilities. Â· Patient education activities are documented to assure continuity of patient education. Â· Maintains department specific educational requirements. Â· Contributes to learning/educational experiences of students and nursing staff members by serving as a resource professional. PROFESSIONAL DEVELOPMENT Â· Provides leadership to members of the nursing care team. Assures consistent application of Universal Precautions. Â· Participates in staff performance evaluations, including self, peer and others as requested. Â· Evaluates own professional practice, develop goals, establishes a plan and reviews progress toward goals, making revisions as indicated. Participates in informal/formal continuing education activities relevant to goals. Â· Is flexible and willing to accept changes in patient care responsibilities and systems of nursing practice and care delivery. Â· Maintains awareness of current healthcare and public policy issues which impact or have potential to impact nursing practice and/or patient care. Â· Serves and/or supports hospital and/or nursing unit based committees as a means of communicating the goals of nursing service and improving the practice of nursing. Â· Attends and participates in staff meetings. Reads and initials meeting minutes reflecting accountability for information presented at meetings not attended. Â· Practices professionally under the guidelines outlined in the Munson Medical Center Registered Professional Nurse position description. ADDITIONAL RN CHART REVIEW RESPONSIBILITIES Â· Completes past testing/information research on next day nurses chart or on charts of patients that have not accessed ARTC. Â· Reviews previous day’s patient appointment charts, including testing results and report abnormals to physician’s office as outlined in the testing report guidelines. Â· Completes 24 hour review of Eye Center charts and delivers charts to department with a status report utilizing the chart log. Â· Completes review of surgical charts, including test results, orders, appropriateness of admission, and for completeness of information. Â· Completes U.R. sheets for next day surgical/CCL charts. Â· Performs an on-going review of all incoming testing and Histories and Physicals before placement of information on patient chart for next day surgery. Â· Facilitates communication with physicians and their office staff utilizing problem resolution skills. Â· Works closely with the chart prep coordinator and file clerk staff to ensure a complete product on delivery to the various departmental customers of ARTC. Â· Acts as a triage nurse for walk-in patients to ensure appropriate pre-op/pre-procedure testing has been ordered. Â· Acts as a resource for ARTC and other PAS staff. Â· Attempts completion of any uncompleted patient profiles as time allows. Â· Takes verbal orders from physicians and their office staff. Â· Facilitates an appropriate work pace within the chart prep area to ensure timely delivery of charts and information Â· Performs other duties as assigned.
