About Medical Social Worker II


Masters degree in Social Work from a CSWE (Counsel on Social Work Education) accredited college or university OR a bachelor’s degree in Social Work from a CSWE accredited college or university, with a minimum of three years experience in a medical setting, preferably in an acute care hospital.

Must be eligible for licensesure as a Social Worker within the State of Michigan and obtain licensure within a prescribed period of time.


 Reports to the Assistant Manager of Social Services. 


 Cares for patients in the age category(s) checked below: __Neonatal (birth-1 mo) __Young adult (18 yr-25 yrs) __Infant (1 mo-1 yr) __Adult (26 yrs-54 yrs) __ Early childhood (1 yr-5 yrs) __ Sr. Adult (55 yrs-64 yrs) __Late childhood (6 yrs-12 yrs) __Geriatric (65 yrs & above) __Adolescence (13 yrs-17 yrs) X_All ages (birth & above) __No direct clinical contact with patients

  • Uses effective customer service/interpersonal skills at all times.

  • Works collaboratively with a multi-disciplinary team to promote effective, efficient, safe patient throughput. Participates, as needed, in the department’s Code Triage plan.

  • Assist the physicians and other members of the health care team in understanding the significance of social and emotional factors relating to the patient’s health problems. Helping patients and families to understand medical recommendations.

  • Provides social work services to restore optimal social and health adjustment within the patient’s capabilities. Assists patient and families in developing and utilizing effective strategies in coping with acute/chronic illness, hospitalization, and treatment. Interacts with patients and their families in the development of a comprehensive and timely plan for discharge from the hospital.

  • Participates as indicated in patient and/or family conferences. Maintains knowledge of community resources that can be utilized by patients and families in order to assist with re-entry into the community and to maximize independence and health restoration. Seeks to identify and develop resources within the institution and the community to address unmet patient/family needs.

  • Maintains strict confidentiality and respect for patient/family autonomy. Promotes hospital-wide understanding of the social work role, functions, and principles.

  • Presents educational/training programs regarding social/emotional factors of illness their impact upon patients, families, and health care process. Acts as facilitator for educational/support groups within the hospital and in community settings.

  • Assures compliance with certification and regulatory agencies. Maintains competency by participating regularly in staff meetings, journal clubs, and other mandatory and/or optional hospital-based or community educational opportunities.

  • Participates in policy and procedure development, review, and modification relative to the department or social work concerns.

  • Performs administrative tasks related to records, productivity monitoring, quality assurance, etc. Participates in the planning and development of hospital programs and services.

  • Practices in accord with NASW Code of Ethics.

  • Performs other duties and responsibilities as assigned.
