About Clinic Coordinator LPN

SUMMARY

The LPN provides patient care and clinical leadership to the Advanced Wound Center clinic. This person ensures that patient care is efficient and coordinated. They assist department leadership in improving processes, upholding quality, and growing the program to meet community and organization needs.

ENTRY REQUIREMENTS

Graduate of a recognized and accredited Practical Nursing program with current Michigan license and in good standing with the State of Michigan Board of Nursing.

Three years healthcare experience with increasing responsibility.

Meets pharmacology requirements in accordance with Munson Medical Center’s general nursing policy.

Knowledge of bookkeeping, insurance and coding systems required.

Possesses current BCLS or is able to obtain within 60 days of employment.

Has experience in a hospital or physician office setting.

Demonstrates excellent interpersonal, communication and organizational skills. Must possess the ability to work independently while maintaining a team effort.

Possesses ability to multiple tasks and duties and maintain a calm professional manner in a busy office setting.

ORGANIZATION

Receives direction from the office coordinator. Will also receive direction and supervision from the Director of Outpatient Services. Functional supervision is exercised over subordinate staff.

SPECIFIC DUTIES

  1. Supports the Mission, Vision and Values of Munson Healthcare.

  2. Embraces and supports the Performance Improvement philosophy of Munson Healthcare.

  3. Demonstrates adherence to all policies and procedures.

  4. Maintains a professional working relationship with other hospital departments, patient’s families and other health care professionals.

  5. Acknowledges that safety is self-responsibility. Knows physical requirements of the job and works within those guidelines. Performs job duties safely at all times, utilizing learned body mechanics and transferring/lifting techniques. Plans actions to promote safety. Reports any unsafe situation/equipment according to hospital procedure.

  6. Assists clinical and administrative leadership in implementing process and policy changes to improve efficiency and ensure compliance. Evaluates resource utilization and makes recommendations on processes, vendor relationships, etc. to ensure proper patient care coordination and efficiency.

  7. Provides clinical leadership and direction to technician staff. Leads clinic functions to support needs of providers and therapists.

  8. Serves as “point” person (first contact for problem resolution) for all office staff and practitioners.

  9. Communication liaison between physician(s) and staff, and between staff and manager.

  10. Trains staff in office processes.

  11. Assists physician(s), staff and manager with annual performance reviews for non-physician staff.

  12. Suggests office process improvements to physician(s), staff and office manager; assists with process improvement implementation.

Clinical Duties

  1. Assists providers and physical therapists with patient care. Duties include, but are not limited to, patient intake, transfers, positioning, preparation for visit, assisting with procedures, taking vital signs, documenting clinical information, taking clinical photographs, re-dressing, assisting with patient exits, and room turnover.

  2. Acting as an advocate for the patient in his/her interactions with health care services, including coordinating with other services (like home care and home medical equipment) to address patient needs efficiently.

  3. Participates in patient education activities.

  4. Performs phone triage with patient’s clinical concerns and refers questions to physicians when appropriate. Responsible for accurate documentation of care and phone calls.

  5. Managing Inventory/Stocking rooms

Administrative Duties

  1. Backing up/Assisting clerical and technician staff with administrative duties, including, but not limited to, Answering phones, Scheduling appointments, Processing referrals, Checking in and checking out patients, Assembling charts, and Entering charges into STAR

  2. Using computer applications

  3. Attending staff meetings and development programs.

  4. Directing own continuing education activities and keeping skills current, expanding knowledge base and complying with recertification/registration requirements as necessary.

  5. Performing all duties listed in department specific competencies.

  6. Perform other duties and responsibilities as assigned.
